Nurse Case Manager on Hawaii Pathways Project Team collaborating with adult clients experiencing chronic houselessness and mental health/substance use issues and providing wrap around housing and case management services. Collaborative team member on interdisciplinary teams in various job positions with local nonprofit agencies.
