The purpose of this EMDR Follow Up Case Presentation Form is to support the consultee in providing an updated summary of a previously presented case for continued discussion during the consultation process. This form allows the consultee to outline clinical progress, challenges, shifts in case conceptualization, and the integration of prior feedback. The consultant can then offer further guidance, clarification, and EMDR-focused recommendations based on the client’s evolving presentation. Both consultants and consultees may use this form as provided, adapt it as needed, or incorporate additional documentation tools to support meaningful and focused consultation.
